First customize different Intake Form Layouts for the different types
Click the gear icon
Click Setup
Search "Object Manager"
Click Object Manager
Search "Intake"
Click Intake Form
Click Record Types
Click New
Enter the Record Type Label
Click Next
Select the Layout for this Record Type
Click Save
