Intake Form Packages are a combination of intake form type (ie. Acupuncture, Chiropractic) paired with signed forms.
Before creating Intake Form Packages, you will need to create Intake Form Types and Signed Forms

Click the App Launcher icon
Search Intake Form Packages
Click Intake Form Packages
Click New button
Select Intake Record Type
Select Signed Forms
Click Create
Assign to a Service
