Go to the Reports Tab

Click New Report

Expand the Other Reports folder

Find and click the SOAP Notes report type, then click Create

Choose Show All Soap Notes

Choose Session Start Date Field

And a time frame you would like to report on

Drag and Drop fields you would like to see in the report

Click Run Report

When satisfied, you can click Save

Name and save the report
