Go to All Tabs (+ sign)

Go to Configuration Center tab

Click on the Intake Form Configuration tab

Fill in the desired form title and text - You can copy and paste form text from your current PDFs or Word documents.

Use special tags for special fields:

"{Contact.Name}" - Inserts Contact Full Name

"{Contact.FirstName}" - Inserts Contact First Name

"{___}" - Inserts Text field

"{_x_}" - Inserts Checkbox

After filling out all the forms, scroll to the end of the Intake Form Configuration Center and click "Save Intake Form Config" button. (Save frequently so the browser doesn't timeout on you!)
